Strange happenings with Avatars

I am seeing very strange things happening with the avatars - they are all mixed up!
Some people have other people's avatars next to their names and my avatar is also appearing along side names other than my own. I have cleared my cache which helped for a day but it has started happening again. A right old mix up and very confusing. Anyone else seeing this? Anyone know how to solve the problem?

@happysnap I saw it one time and thought I just needed more sleep! I haven't seen it since but haven't looked today. Mine went away on its own!

Wow! bizarre! Only Avatar mix up I notice is that I do not always see people's new avatars as they update them.
