In case anyone is interested, this is a piece from Blenko glass company in Milton WV. Specifically a Large Dimple Glass in Cobalt Blue. This is "Crackle" glass, where they immerse the molten glass in cold water to cause the cracking, the reheat to seal it. It is really rather cool....
