Not specifically, but I have wondered the same thing (esp re Model Releases). I ended up googling MR + local professional bodies, MR + specific type of photography I am doing etc, finding a bunch of different stuff, downloading a couple of samples and making up my own as some kind of combination of different stuff I'd found. I'm not entirely sure how legally binding it is but I figure something is better than nothing? What type of photography is it?

Hi Pea. If you send me an email to eve.pulsater@gmail.com I can send you an example of a model release form that I was given earlier in the year when I did a portrait / boudoir shoot with two professional models (picture at http://365project.org/creampuff/365/2014-06-05 ). The only thing I will say is that it is for a model 18 years and over. @deeganp
@creampuff i dont think thats what @deeganp needs. Your model release form is questionable as to whether you actually need it especially in the UK - but gives you the rights to use the images.

@slava on here shoots professionally and she rocks, may be worth asking her or getting her input?
The only bad judgement call on Slava's part if wanting to shoot male boudoir - or "moudoir" as i've dubbed it :)
