@aponi - Are you setting up a folder first that's 2016/November? Then when you import your files, do you have "copy" selected (i.e., that you want Lightroom to copy the files from the card/camera to somewhere else), plus the Destination option selected to organize by date with the month/year selected, and then you select the folder you have created first as the destination? I know that will give you what appears subfolders.
I organize all my files by date, though I use the day format instead of the month. I leave the block "into subfolder" unchecked, select organize by date, date format (in your case select month/year format) and then select your "Pictures" folder from the options (I keep all my photos on an external drive, so I select the drive). It will then create a new folder only if the folder doesn't exist. If the folder exists it will put the files in the existing folder.
@aponi I think @fpmtngal might have the answer - uncheck the subfolder option & see if that helps. (even though that seems counter intuitive, it worked for me in the past & does still create new folders as needed)
I store my images in individual date folders within the year parent folder (ie not by month) so have not tried your system. To view all images by month in Lightroom I create a collection as needed, or use the smart collection to view those images taken in the 'last month'.
