What @gurry said. I always shoot mine from the steps of the church or some such place, looking down at the guests. It doesn't prevent it, but it reduces the instances. I also do some variants, and invariably ask the bride to either put down her bouquet or to raise her other hand - the flowers are the worst trap for this. But in this shot, people are hidden behind people, pillars etc, indicating the shot was not set up correctly to start with, so was never going to be right.
