I've seen a few posts recently about people having trouble with out of focus photos. The following article gives some information, advice and tips on how to achieve better focus. It's mainly aimed at using AutoFocus but there's a few tips that can be applied to manual focus too.

Yay - thanks for this ... had to sit in a boat this afternoon bobbing up and down in a hefty NorEaster taking 'action' shots of a speeding windsurfer! Talk about needing to be stable ... Upon re reading the article I realised I'd left the Focus Mode on AI Focus instead of AI Servo! doh!

If you'd not posted this I'd be making a similar mistake over the next two days too.
