Smiling Clydesdale?

There was a lot of controversy on facebook whether a Clydesdale "smiling" like this was in pain or not because of the bit in its mouth.

Horses don't smile, but it doesn't necessarily mean they're in pain. They pull faces like this for all sorts of reasons.
