The trick is to use a very wide aperture on your lens. On these two I used f/1.4. You also need to focus on the subject, and have the lights in the distance; out of focus. With my f/1.4 lens I only needed the subject to be about 18 inches / 46 cm from the lights. With a smaller aperture, say f/1.8 or f/2.8 the lights would need to be farther away. Assuming the lens stays the same focal length. In this case it was my 50mm lens.
You can get a similar effect with a longer lens, and have the subject a little farther from both your camera, and the lights.

In this image, I was close to the subject, but the light source was in a field at least 50 paces away from the camera, probably more.
